Enrollment Forms

To be considered for enrollment, an application for admission and the enrollment agreement must be completed and signed by the applicant and parent or guardian (if applicable) and submitted to The Art Institute of California - Hollywood. We also require proof of high school graduation or GED scores. In addition, prospective students are strongly encouraged to visit the school, although a visit is not a condition for submitting the application for admission or enrollment agreement. Arrangements for an interview and tour of the school may be made by contacting the Admissions Department.
