To order a transcript from the registrar's office, download the form below, fill it out completely, and fax it to the registrar's office at 770.394.9800. There is no charge for transcripts. You need to complete one form per individual or institution if transcripts are being sent to multiple addresses. Please allow two weeks for processing your request. Emailed requests for transcripts cannot be accepted.
