The forms below are to be completed online by the student's parent and electronically signed and submitted. When the parent clicks on one of the online forms for the first time, they will be required to "Create an Account". THIS IS A SEPARATE ACCOUNT FROM THE STUDENT ACCOUNT! The parent will want to remember the password they choose, as they will need it to log into the online forms in the future.

• Parent FFELP Recommended Lender List - Complete only if you are borrowing a Federal PLUS loan. After making your lender selection, will need to complete a Master Promissory Note (MPN) on the lender's website.

• Parent Authorization form - Complete only if you are borrowing a Federal PLUS loan.

• Certification of a Parent Not Borrowing a Plus loan - If you are not borrowing a Parent PLUS loan for your dependent student, your Financial Aid Officer will inform you if this form needs to be completed.
