The forms below are to be completed online by the student and electronically signed and submitted. When you click on one of the online forms for the first time, you will be required to "Create an Account". You will want to remember the password you choose, as you will need it to log into the online forms in the future.

• Student FFELP Recommended Lender List - Complete only if you are applying for a Federal Stafford Loan. After making your lender selection, will need to complete a Master Promissory Note (MPN) on the lender's website.

• Master Promissory Note - Once you complete the Stafford FFELP Recommended Lender List, open the PDF. Click on the printed web address of the same lender you specified on the Stafford FFELP Recommended Lender List. Then follow the on-line instructions to apply for a Federal Stafford Loan.

• Supplemental Data - Students borrowing Federal loans must provide references that can help us get in touch with you in case you forget to provide us with updated contact information.

• Student Authorization Form - Complete if you are receiving financial aid.

• Student Private Loan Recommended Lender List - Complete only if you are applying for an Alternative loan. You should meet with your Financial Aid Officer to review your financial aid eligibility prior to applying for an Alternative loan.  (**Your credit will be checked to see if you are approved for this loan.)
