The forms below are to be completed online by the student and electronically signed and submitted. When you click on one of the online forms for the first time, you will be required to "Create an Account". Colorado Residency Criteria and Questionnaire for Dependent Students - Complete this form if you and your parent(s) are a documented Colorado resident. (**Student - before completing of this form you will be required to enter your parent's name and e-mail address. This form will then be emailed to them for completion of their information.)

Student Private Loan Recommended Lender List - Complete only if you are applying for an Alternative loan. You should meet with your Financial Aid Officer to review your financial aid eligibility prior to applying for an Alternative loan.
