Enrollment Form

Ready to apply or enroll at The Art Institute of Phoenix? Here you'll find the necessary forms. Fill out an Application for Admission and an Enrollment Agreement and send it to The Art Institute of Phoenix with a $50 application fee. We also require a $100 tuition deposit, due within 10 days after you’ve submitted the Agreement, and proof of your high school graduation or a GED.
