Disability Services

The Art Institute of Phoenix provides accommodations to qualified students with disabilities in accordance with Section 504 of the Rehabilitation Act and The Americans with Disabilities Act as Amended. Student Affairs at Ai Phoenix now partners with Disability Support Services (DSS) to co-manage the accommodations process, assist qualified students with disabilities in acquiring reasonable and appropriate accommodations and in supporting equal access to services, programs and activities at The Art Institute of Phoenix. In addition to support from DSS, I am also available to help you here locally.

If you have a disability and would like to request accommodations, notify the Disability Support Services staff of your specific limitations and, if known, any specific accommodations. You will be asked to supply documentation of the need for accommodation. Classroom accommodations are not retroactive, but are effective only upon sharing the Notification of Accommodations Letter provided by DSS with the instructor. Therefore, you are encouraged to request accommodations as early as feasible with DSS to allow for time to gather necessary documentation.

Below, you will find the Disability Notification Form and the Guidelines for Documentation of a Disability. Upon the receipt of the Notification Form, a Disability Services Coordinator will contact you regarding your documentation to determine eligibility for reasonable accommodations at The Art Institute of Phoenix. If you have documentation, you may fax or mail it to the address below along with the Notification Form. PLEASE MARK CONFIDENTIAL ON YOUR FAX COVER SHEET OR MAILING ENVELOPE.
