Your name and maiden name if appropriate
Your full address
Your phone number AND email address
Your graduation date and course of study
If you are a current student or did not graduate, include the month and year you started at The Art Institute.
The name of the friend you are searching for and any information you might be able to contribute such as department of study, start date, graduation date, and hometown.
A note to the friend so that they know why I am forwarding your message to them. This is very important.
