Request a Transcript

To order a transcript from the registrar’s office, download the form, fill it out completely including student signature. You can fax the form to 503.228.2895, and mail in the fee; or mail form with fee to: Registrar’s Office, Attn: Transcripts, 1122 NW Davis St., Portland, OR 97209-2911. The fee is $3.00 for each transcript ordered. Provide the complete address of each recipient. Please allow one week for processing your request. Emailed requests cannot be accepted.
